Glossary
Gonorrhea (gonococcal urethritis)
After an incubation period of 3 to 7 days from the time of infection, pain when urinating and discharge of pus from the urethra. In men, symptoms often appear 2 to 5 days after the time of infection, but sometimes symptoms do not appear until 30 days later. [Men] ・Feel a slight itching or heat in the urethra. ・Yellowish pus comes out of the urethra. ・Feel strong pain when urinating. If left untreated, you may experience frequent urination, difficulty in urination, and bleeding at the end of urination, and inflammation may spread to the prostate and epididymis, and may cause infertility. [Women] ・Symptoms such as pain when urinating and frequent urination may be so mild that the person herself may not notice. If left untreated, the infection may progress to the uterus and fallopian tubes, causing pelvic infection and infertility. ・Increased vaginal discharge. It is said that pharyngeal gonorrhea is transmitted to the pharynx (throat) through oral sex. When the gonorrhea bacteria (gonococcus) infect the pharynx, symptoms such as swelling, pain, and fever in the pharynx may occur, but there are also cases where no symptoms are present. Treatment for gonorrhea, whether it is the urethra or pharynx, can be completed with one week of oral antibiotics or one infusion. Oral antibiotics may be ineffective if there is a possibility of resistant bacteria infection, in which case intravenous treatment is used.
syphilis
It can be detected by a blood test. It will not test positive until 3 weeks after infection. Symptoms First stage (from 1 weeks after infection) - Painless red lumps the size of a bean to a pea appear on the infected areas such as the genitals, anus, and mouth. This becomes latent, but improves naturally in 3 to 4 weeks. In most cases, women do not notice it. Second stage (from about the end of the first stage) - Red spots appear scattered on the skin of the whole body. - Papules (raised bumps on the skin) and hair loss are seen on the back of the head. - There is no itching or pain, and if left untreated, the symptoms will disappear naturally in 6 to 2 weeks. After the end of the latent stage and second stage, the patient enters an asymptomatic latent stage that lasts from several weeks to several years. During this period, it can only be diagnosed by blood test. Tens of percent of these progress to the latent stage. Late stage (from about 1 years after infection) - Lesions called gummas (hard lumps or bumps appear on the skin and internal organs, destroying the surrounding tissue and leaving scars when healed) appear.・Arthritis and loss of feeling in the hands and feet. ・Damage to the heart, blood vessels, brain, etc., making it impossible to carry out daily life. Syphilis treatment ●Administration of antibiotics As a rule, intravenous drips are not used, and treatment is by oral medication only. Early syphilis is easily treatable. In the early stages of drug administration, patients may develop a fever of around 2 degrees. Even if symptoms disappear, there is a risk of recurrence, so treatment is continued until the pathogen is eliminated. (6 to 2 months)
Chlamydia
It is the most common sexually transmitted disease. Women often show almost no symptoms, and it can cause infertility in both men and women. The incubation period is 1 to 3 weeks. Mild urethral pain, discomfort, and discharge from the urethra. [Men] - About half of the patients experience difficulty urinating and pain when urinating. - A creamy pus comes out of the urethral opening, which is thin at first and gradually thickens. - It may also cause prostatitis and epididymitis. [Women] - About 2% experience increased vaginal discharge and lower abdominal pain. - The symptoms are extremely mild, with only one in five infected people showing symptoms. - As it progresses, inflammation of the uterus and fallopian tubes progresses, and it may cause infertility. Treatment for chlamydia Treatment is completed with one oral dose. Oral antibiotics are used as the first choice of medication. A recurrence test for chlamydia is performed one week after administration, and if the test is negative, it is considered to be "completely cured."
Azalea Condyloma
The main causative viruses are HPV types 6 and 11. Bumps on the glans and foreskin. They are caused by a virus and gradually increase in number. Treat with antiviral ointments. There is no pain or itching. It is necessary to treat it immediately as it can be transmitted to other people. Symptoms There is an incubation period of about 1 to 2 months, and pointed "warts" form on the foreskin and glans. There are no subjective symptoms, pain, or itching, and warts form and grow one after another in a short period of time, and recurrence is often repeated. It is also caused by the fact that the glans is not exposed in many people with phimosis. In rare cases, it can be transmitted from saunas and public baths. Even if infected, it can heal naturally by the autoimmune system in about a year. The incubation period can be as long as 1 to 1 months, and the source of infection may be unknown. Differential diagnosis Some patients who develop warts around the glans ask if they have genital warts, but most of them are called Fordyce spots or pearly penile papules, and are not a disease. Fordyce spots are spherical bumps less than 8mm in size that develop from the penis to the tip of the glans, and are independent of the sebaceous glands. They are seen in 1% of adult men and are a physiological change that is not related to viruses. Pearly penile papules are uniformly sized bumps lined up along the end of the penis (the so-called ridge of the glans). They are a completely harmless physiological change and are seen in 65% of adult men. Fordyce spots are not harmful, but they are often treated because of their appearance. Since they are not a disease, they are not covered by insurance, and the average treatment cost is 20 to 2 yen. Even if they look more like Fordyce spots or pearly penile papules than genital warts, a diagnosis by a specialist is required. The fold-like or papilla-like growths at the entrance to the vagina and its surrounding area (vaginal vestibule), called "vestibulocochlear papillae," are sometimes misdiagnosed as genital warts, but they are a normal change also known as "fairy folds." The distinguishing features are that it occurs symmetrically on the inside of the labia minora, that the nipples are regular and blood vessels are not visible inside. Examination Generally, there is no special examination for genital warts, as they mainly infect the mucosal epithelium, but since it can also be combined with syphilis and other sexually transmitted diseases, a general sexually transmitted disease examination is recommended. [Men] - Many warts appear on the glans, coronal sulcus, inner and outer foreskin, and scrotum of the penis. [Women] - Many warts appear on the labia majora and minora, vaginal vestibule, perineum, vagina, and cervix. Treatment methods - Drug treatment and surgery Liquid nitrogen freezing surgery Electric scalpel excision surgery Laser light vaporization surgery There is a risk of infecting your partner, and symptoms often last for a long time with repeated recurrences and increases and decreases, so it is best to receive treatment. Until now, cryotherapy, surgical excision, and vaporization treatment were the main treatments, but treatment with topical medication is now possible. It is a drug that treats by activating the immune system, and is thought to suppress viral proliferation by promoting the production of cytokines and to damage virally infected cells by activating the cellular immune response, utilizing the patient's own defense mechanism against viral infection to eliminate the lesions. It takes more than a month to heal, but it is an effective method. If treated properly, the recurrence rate is low.
Genital herpes
It is caused by the herpes virus, and may cause a fever in the case of initial infection. Women are more likely to develop severe symptoms, so immediate treatment is required. In men, small blisters appear on the penis, which then become ulcerated, but unless the medication is taken immediately after the onset of symptoms, no effect can be expected. Symptoms - Red bumps, blisters, and ulcers (skin sores) appear on the genitals. - When infected for the first time, it is often accompanied by severe pain and fever. - In the case of recurrence, symptoms are often mild, with small blisters and ulcers. Treatment There are three main antiviral drugs to treat the symptoms of genital herpes. Preventive treatment with oral and topical medications is also possible.
Hepatitis B
Hepatitis B is transmitted through blood, such as sexual intercourse or blood transfusions, and from mother to child. The infection rate through sexual intercourse is said to be higher than that of the AIDS virus. Hepatitis B is commonly found in Asia and Africa, so there are many cases where people contract the disease overseas and bring it back to Japan. There may be no clear symptoms, but a blood test can reveal the details of the infection. Symptoms: - Fatigue and tiredness, as well as insomnia, loss of appetite, nausea, slight fever, diarrhea, etc. - Dull pain around the liver. - If the symptoms progress, it may lead to cirrhosis or liver cancer, so caution is required.
Hepatitis C
Hepatitis C is caused by the hepatitis C virus. It is mainly transmitted through blood contact, but can also be transmitted through mucous membrane contact such as sexual intercourse. It is not as contagious as hepatitis B. There are often no symptoms, so you cannot know if you are infected without being tested. Symptoms: General fatigue, slight fever, loss of appetite, jaundice, dark urine.
Hair lice
Hair lice prefer apocrine sweat glands and live in the pubic hair area. They suck blood several times a day and lay 1 to 1 eggs a day. The eggs are laid on the pubic hair and are observed as white dots. If the adult does not suck blood for 2 hours, it will die, but the eggs can live for 3 days. Hair lice that infest the hairline cause intense itching when they suck blood, and blood dots appear on underwear. Symptoms: Itching in the area where hair lice are infested. Treatment: Shaving the hair in the infected area. Hair needs to be shaved repeatedly according to the egg hatching cycle. Lice eggs hatch for about a week, so the hatching period is repeated 24 to 10 times every 1 to 3 days.
AIDS (HIV)
AIDS is an abbreviation of Acquired Immune Deficiency Syndrome. When you are infected with the AIDS virus (HIV), your body's immune system is destroyed, so even weak pathogens such as mold, bacteria, and viruses can make you sick. HIV can also cause symptoms similar to encephalitis. Because your immune system is weakened, you cannot suppress the symptoms caused by weak pathogens that you were infected with unknowingly as a child, and you become sick. HIV is found in the blood, semen, vaginal secretions, and breast milk of infected people. Blood infection is transmitted by blood transfusions and sharing of syringes, and semen and vaginal secretions are transmitted through sexual intercourse. HIV is also found in urine and saliva, but it is not transmitted because it is in small amounts. HIV has limited routes of infection, so if you understand how it is transmitted and take care, you will not infect others in your daily life. About symptoms: In the early stages of infection, you may experience cold-like symptoms, but in many cases you will not notice any symptoms for a long time (on average about 10 years). You will not know if you are infected unless you get tested.・ After a period of asymptomatic illness, symptoms such as rapid weight loss, excessive night sweats, and diarrhea may appear. ・ As the disease progresses, the immune system weakens, and the patient may become susceptible to infections and malignant tumors that would not occur if healthy.
Candita
Candidal vaginitis is caused by the proliferation of mold. It is a disease caused by the proliferation of Candida albicans (a fungus or mold) in the vagina, which everyone has. It is a parasitic fungus that lives in the intestines and is mixed with feces, from which the bacteria often attach. It is said that about 1% of people have Candida in their vagina. When you are healthy, you will not develop the disease even if the bacteria attaches to your body, but you are more likely to develop it if your immune system is weakened due to fatigue or pregnancy, or if you have diabetes or have taken antibiotics for a long period of time. It is more likely to develop when your body is abnormal, such as when the seasons change, when you are taking antibiotics, when you are not feeling well, or when your immune system is weakened, and it is accompanied by itching of the vulva. It can also be transmitted through sexual intercourse. Symptoms: A white, cottage cheese-like, crumbly vaginal discharge. Inflammation occurs and you feel a strong itching. As the symptoms progress, a large amount of vaginal discharge comes out, the vulva becomes red and blistered, and severe itching occurs. Treatment for women Vaginal tablets, ointments, creams, and oral tablets are prescribed as treatment. Treatment will continue for about a week, and if the effect is not sufficient, additional treatment will be performed or other causes will be examined. Treatment for men Keep the genitals clean and apply ointments or creams. Men are less likely to suffer from candidiasis, but the infection rate increases if the patient is uncircumcised. If the symptoms recur, circumcision surgery can improve the condition.
Trichomonas
It is transmitted through normal sexual intercourse, but while Trichomonas is very susceptible to drying, it is infectious for a long time in water, so it is known to be transmitted on toilet seats and in baths. Incubation period The incubation period for Trichomonas infection is said to be as early as about 3 days, as late as one month, and on average around 1 days. If there are no symptoms, the incubation period may not be clearly identified. It is highly contagious, and there is a high possibility of infection with one sexual intercourse. It is said that about 10-20% of women are asymptomatically infected, but one-third of them show symptoms within 50 months, so there is a wide range of incubation periods. If you are infected with Trichomonas protozoa, you may transmit it to your partner through sexual intercourse even if you do not have symptoms. It has been revealed that the increase in vaginal trichomoniasis, especially among young women, is due to sexual intercourse during the incubation period. Because it is highly contagious, you can become infected by sharing the same bath, towel, toilet, etc. as an infected person during the incubation period. [Men] - Most cases are asymptomatic, and even if symptoms are present, they are mild and include discomfort during urination. [Women] - A foul-smelling white or yellow frothy or pus-containing vaginal discharge. - Severe itching in the vulva. - Pain during urination or sexual intercourse may occur. - In severe cases, the vulva may become red and inflamed. Treatment - Oral medication is prescribed.
