There are self-proclaimed charismatic cosmetic surgeons who, despite being charismatic, cannot perform any surgery. A cosmetic surgeon is a cosmetic surgeon if he can perform surgery. If he cannot perform surgery, he is not a cosmetic surgeon. Please do not be fooled by the word "charismatic."
